D) Vitamin K, hold Coumadin, recheck INR in 8-12 hrs. Rechecking at this time frame would be the most appropriate.


A) Protamine sulfate is actually a reversal agent for heparin overdosage, not Coumadin

B) An INR of 8.5 is supratherapeutic. Ideally for this patient their INR should be between 2-3

C) If the INR is >5 Generally Vitamin K is given. If concerns of active bleeding and INR high, FFP may also be given as well.
